We would rather say that plainly than put a form here, take what you told us, and have nowhere safe to keep it.
An application asks people to share difficult things — health, money, family, sometimes things they have told nobody else. That deserves somewhere properly secure to live, with a named person responsible for it and a limit on how long it is kept. We are building that. Until it exists, this form stays shut.
